A common question is: What is MyChartCentral & Lucy? MyChartCentral provides you with one main access point for all your MyChart accounts by using one single username and password to manage them all.

Lucy is a benefit and not the same. This is a personal health record that stays in a permanent place. In it you can organize all your medical information, regardless of where it came from. You can request an updated copy of your Epic medical record at any time and store it in Lucy.

With Lucy, you can also add personal information about your health and choose to share it with other facilities where you receive care. If you change your insurance or you move away and need to leave your current organization, Lucy and all the information stored there will follow and stay with you.

Follow these easy steps below if you are a NEW patient who needs to register and create a new account to manage your information:

  1. Visit this link on your device in an internet browser.
  2. Click on the “Activate Now” button.
  3. Type in your Activation Code in the fields provided.
  4. Type in your Date of Birth in the fields provided.
  5. Type in your contact number in the field provided.
  6. Click on the “Next” button.
  7. Proceed to move forward and provide all the required information to complete the registration for an online account.

No Activation Code? Lost / Expired / Never Received?

After you receive your one-time-use access code you will need to use it immediately to activate your account because the code is only valid for 90 days.

A representative can send a new activation code if you have lost it, let it expire, it does not work or did not receive it. Call the number provided at the beginning of this article OR after STEP TWO from above click on the “Request Online” button and then select the “For myself only” sign up form and provide all the required information.

Follow these easy steps below if you are a NEW patient who needs access to the Proxy Parent / Legal Guardian online form:

  1. Visit https://mychart.ssmhc.com on your device in an internet browser.
  2. Click on the “Sign Up Now” button.
  3. Select the correct Proxy form you need such as:
    • On behalf of a child under 18
    • On behalf of an adult family member
  4. Proceed to move forward and provide all the required information to complete the registration for an online account.

Steps if you are a CURRENT patient looking to sign into your current account online:

  1. Visit: www.ssmhealth.com/mychart on your device.
  2. Click on the “LOGIN TO SSM HEALTH MYCHART”.
  3. Type in your username and password in the fields provided.
  4. Click on the “Log on” button and begin managing your current online account.
