Steps for Parents or Guardians to create a MyChart account for patients ages 12-17 years of age:

Follow these easy steps below to create NEW patient account to manage medical information:

  1. Visit this link on your device in an internet browser. Click on the “MyChart for Teens & Parents/Guardians” to review the requirements and information.
  2. As a teenager, you will first need to request a separate / different Activation Code. Click here to do so. As a Parent or Guardian you will first need to fill out and submit an Access to My Family’s Records Consent form and then:
    • Fax it to: (916) 734-2126 OR
    • E-mail it to: HIM@ucdmc.ucdavis.edu OR
    • Mail it in to: UC Davis Health System, Health Information Management Medical/Legal Release of Information Unit 2315 Stockton Blvd., Building #12 Sacramento, CA 95817
  3. Proceed to move forward and provide all the required information to complete the registration for an online account.

For Children of Birth to 12 Years of Age: Parents and legal guardians are provided with full access to their child’s MyChart account. You will be notified fifteen days before your child’s 12th birthday with an alert informing you that your access will be transitioned to a medium proxy access the day before your child’s birthday.
For Children ages 12 to 17 : Parents and legal guardians are provided with medium access to their child’s MyChart account. It will be fifteen days before the child’s 18th birthday that you should receive a notification alerting you that access will be terminated the day before your child’s birthday.
