  4. Proceed to move forward and provide all the required information to complete the request.

Follow these easy steps below if you are a NEW patient who needs access to the Proxy Parent / Legal Guardian online form:

  1. Visit https://mychart.ssmhc.com on your device in an internet browser.
  2. Click on the “Sign Up Now” button.
  3. Select the correct Proxy form you need such as:
    • On behalf of a child under 18
    • On behalf of an adult family member
  4. Proceed to move forward and provide all the required information to complete the registration for an online account.

Steps if you are a CURRENT patient looking to sign into your current account online:

  1. Visit: www.deancare.com/mychart on your device. NOTICE: ONLY BILL PAY is still available on this site until Nov. 1, 2019 for service charges incurred before Sep. 22, 2018. AFTER Nov. 1, 2019 you will need to use the NEW SITE for Dean My Chart. The new site link was provided all the other step guides above.
  2. Type in your username and password in the fields provided.
  3. Click on the “SIGN IN” button and begin managing your current online account.
